Gynecomastia Reduction

  • Males with Enlarged Breasts – Patients with enlarged breasts, including excess skin, fat, and/or breast tissue who are bothered by the appearance of their chest are excellent candidates for this surgery.

  • Symptomatic Gynecomastia – Some patients may experience pain or tenderness of the enlarged breast tissue. Removing the breast gland tissue may improve your symptoms. 

  • Stable Weight and Breast Size – Ideal candidates should have a stable weight and breast size. Weight fluctuation before or after surgery will significantly impact your final result, so you want to be stable at your goal weight before proceeding with surgery. Gynecomastia can be normal in teenage years because of hormonal changes and go away on its own. In most cases, surgery is deferred except in certain circumstances.

  • All Underlying Causes Have Been Addressed – Gynecomastia can be caused by a number of medications, endocrine and metabolic conditions, and tumors. It is important that those underlying causes are treated first. Once the underlying cause is treated and your gynecomastia is stable, then you can be considered for surgery.

  • Healthy Patients – Healthy, non-smoking patients without major medical conditions that would make surgery or general anesthesia unsafe are candidates for this surgery. Conditions that pre-dispose to increased complications such as diabetes, vascular disease, autoimmune conditions, immunodeficiencies, etc. are still candidates for surgery but require additional pre-operative counseling and optimization.

Who Is a Candidate?

  • Medical Evaluation – Dr. Chang will review your medical conditions, medications, prior surgeries, and personal and family history of breast diseases, masses, and cancers. Some patients may be referred to their primary care doctor or a specialist to ensure you are medically optimized for a safe surgery under general anesthesia and post-operative recovery. Because this is an elective surgery, some patients may be recommended to optimize certain parameters before proceeding with surgery. This may include weight loss, smoking cessation, nutrition optimization, resolution of an acute illness or condition, etc.

  • Chest Examination – Dr. Chang will perform a physical examination of your chest and breast tissue. This will include measurements of the size, location, and symmetry of the breasts and nipple-areolar complexes. Dr. Chang will assess the skin excess, skin quality, fat excess, breast tissue excess, and nipple-areolar complex position to determine the best surgical approach.

  • Specialist Referral – Because gynecomastia may be the result of a more serious underlying condition, you may be referred to your primary care doctor, endocrinologist, or other medical specialist to help complete a medical workup. This workup would include a more comprehensive physical exam including a testicular exam, blood tests, and imaging studies. This is more important for patients who are seeing Dr. Chang first, as opposed to having been referred for surgery by their primary care doctor. 

  • Clinical Photographs – Clinical photographs from multiple views will be taken of your chest. These photos are used to plan for your individualized surgery and compare before and after surgery. No photographs will be published online without your written consent.



What Happens Before Gynecomastia Surgery?

  • Pedicle – For the vast majority of cases unless you have severe skin excess, the nipple-areola complex is left in place with only minimal adjustment to its position. For patients with significant skin excess, the nipple-areola complex is moved. For cases that need only a mild-moderate movement, the nipple-areola complex can be left attached to the underlying tissues and moved into the desired position. This helps maintain its color, projection, and sensation. 

  • Graft – In cases of extreme skin excess and a large needed movement of the nipple-areola complex, the nipple-areola complex will be removed and sutured back on as a graft in order to maximize the chance that it survives. This will result in some degree of loss of normal pigmentation, projection, and sensation.

Nipple-Areola Complex

  • Periareolar – A periareolar incision is a good option for patients with no skin excess and who have an area of firm glandular tissue right underneath the nipple-areola complex. The incision is blended in the junction between the areola and native chest skin.

  • Circumareolar – A circumareolar incision is a good option for patients with minimal skin excess and/or enlarged areolae to produce some skin reduction. The incision is blended in the junction between the areola and native chest skin.

  • Inframammary Fold – This incision is placed in the fold formed by the lower border of your pectoralis major muscle. The incision is placed here to blend with the natural skin adherence to your chest wall muscles. This incision is the best option for patients with significant skin excess and can be extended to your sides or back in combination with an upper body lift for patients with massive weight loss.

Liposuction is used in most cases of gynecomastia unless your primary problem is excess skin alone without excess fat or significant amount of glandular tissue. Liposuction alone as a technique is a good option for young patients with no skin excess, good skin quality, and just have mildly enlarged gland or fat tissue.
