Causes of Scars

  • Hypertrophic Scars – Hypertrophic scars are raised, red, and thickened scars that develop due to excessive collagen production during the healing process. They usually remain within the boundaries of the original wound but can be unsightly and sometimes itchy or painful.

  • Keloid Scars – Keloid scars are characterized by an overgrowth of scar tissue that extends beyond the original wound site, forming a raised, often firm lump on the skin. They can be itchy or painful. They are more aggressive than hypertrophic scars and can continue to grow over time. Keloids are more common in individuals with a genetic predisposition.

Technique

  • Local Anesthesia – For small skin cancers that need more routine reconstruction techniques, the procedure can be performed under local anesthesia in the office.

  • General Anesthesia – For more complex skin cancers that require more advanced reconstruction techniques, Dr. Chang may recommend that the procedure be performed in the operating room under general anesthesia. 


Anesthesia 

  • Steroid Injection – Corticosteroids, such as triamcinolone acetonide, are injected directly into the closed incision where the prior scar was. The steroid injections can also be repeated post-operatively.

  • 5-Fluorouracil (5-FU) Injection – 5-fluorouracil is a chemotherapy agent that, when injected into the keloid, works by targeting and disrupting the growth of fibroblasts, the cells responsible for collagen production.

  • Radiation Therapy – Radiation therapy is a powerful treatment option for managing keloids, especially when other methods have not been successful. Post-surgical radiation involves delivering a controlled dose of radiation to the keloid area to inhibit collagen production and prevent further keloid growth. This treatment is typically administered in a series of sessions over a few weeks. Radiation therapy is usually considered for more severe or recurrent keloids and is combined with other treatments to enhance efficacy.
